Welcome to the Navigating the System module! In this session, we’ll explore HIV case management and how it can help you obtain medical and social services.

To help you stay healthy, case managers assess your medical and social needs, and connect you with appropriate services. Together, you and the case manager create a mutual service plan to access resources that meet your needs. In short, case management is a team effort.

To start off, we’re going to give you some background about what case management is and does.
