Referrals to Services
Based on the information you provide, your case manager will give you referrals to needed services. Examples of these services include medical care, mental health counseling, job training, housing, legal assistance, and substance use treatment.
Some services with limited eligibility, including medical transportation, emergency financial assistance, and special housing programs require that you have a case manager in order to access them.
