AFC Releases Request for Proposals

The AIDS Foundation of Chicago (AFC) is announcing the availability of case management and treatment coordination funding. Approximately $4.3 million will be available starting March 1, 2008 for Ryan White Part A and Part B case management services provided through the Northeastern Illinois HIV/AIDS Case Management Cooperative. In addition, approximately $1.7 million is available annually to support Division of Rehabilitative Services (DRS) Home Services case management. AFC is also releasing approximately $600,000 to fund a new component to the case management continuum, Treatment Coordinators. In light of growing nationwide recognition that case management has been identified by HRSA as a “core clinical service” that facilitates linkage to and maintenance of primary medical care services, AFC and the Chicago Department of Public Health (CDPH) have taken the leadership role in developing a new system of case management services that responds to changes in Ryan White Part A and Part B, is responsive to the levels of need of clients in the Chicago EMA, and maintains client access to benefits including transportation, emergency financial assistance (EFA), and emergency housing assistance (EHA). This new system emphasizes treatment and appointment adherence, facilitates active participation in primary medical care and other core clinical services, and monitors health outcomes with the goal of supporting clients as they become partners in their own care.

The purpose of this Request for Proposals (RFP) is to conduct a competitive rebidding process for the newly revised system of HIV case management services in the Chicago area. Part A funds are awarded by HRSA directly to CDPH, who has delegated to AFC the responsibility of coordinating and administering the Northeastern Illinois HIV/AIDS Case Management Cooperative. This bidding process has been developed in accordance with guidelines set out by CDPH, and with the cooperation and support of the program and contracts staff at the Illinois Department of Public Health (IDPH), the Illinois Department of Human Services (DHS) and the AIDS Administrative Unit (AAU).

All agencies interested in providing case management or treatment coordination must submit an application. Please refer to the RFPs for a complete overview of the funding guidelines, grant requirements, and due dates.
