How to Find a Case Manager The HIV case management system links individuals and families with a wide range of necessary services and available resources. The only HIV care system of its kind in the country, the case management network also provides emergency financial assistance for utilities, emergency financial assistance for housing, food vouchers, and transportation services such as transit vouchers and taxi service for people living with HIV.Here is the process for obtaining case managment services: Contact the AIDS Foundation of Chicago at (312) 922-2322 and say that you need a case manager. One of AFC's program staff will refer you to an agency in your area that will provide you with a case manager who can help you get the services you need. Please note: Due to confidentiality, only the person seeking services can enroll as a client in the case management system. Family members, friends, or social workers cannot enroll someone else for services without a formal verbal or written consent. Case managers help people living with HIV obtain:
Some services, such as emergency client assistance and HAP rent subsidies are only available through case managers.
