Advocate Metro Outreach Deaf and Hard of Hearing Program - to implement and evaluate a culturally competent HIV/AIDS continuum of care and prevention services for the deaf and hard of hearing. $5,000
Agape Missions, Inc. - for general operating support of this multi-service AIDS service provider. $15,000
AIDS Legal Council of Chicago - to provide legal services for people living with HIV. $30,000
AIDSCare - for general operating support of this residential program serving people living with HIV/AIDS. $10,000
Asian Human Services, Inc. - to support the African Refugee AIDS Project, which will provide HIV prevention education to recent African immigrants. $15,000
Aunt Martha's Youth Service Center - to provide HIV prevention education and anonymous STD testing to youth and young adults in southern Cook County. $10,000
Better Existence with HIV - to enhance and expand the Prevention for Life program targeting people living with HIV. $20,000
Bonaventure House - to support the Occupational Therapy Program, which helps people living with HIV develop or regain employment and life skills. $5,000
Bridging the Tys to Jordan - for general operating support of Jordan's Place, a residential facility serving people living with and at-risk for contracting HIV. $10,000
Catholic Charities of Lake County - to support the opeartion of the food pantry that serves HIV-positive clients. $5,000
Centro Communitario Juan Diego - to support the HIV/AIDS Prevention Health Promoter training program targeting Latinas. $10,000
Centro Romero - to support the HIV/AIDS awareness program for adolescent Latinas on the north side of Chicago. $10,000
Centro San Bonifacio - to provide general operating support for the Community Health Promoters program, a peer-led HIV prevention education effort. $10,000
Chicago House & Social Service Agency - to support the activities of the Project CHOICE Prevention with Positives initiative targeting the residents of Chicago House. $15,000
Chicago Recovery Alliance - to purchase sterile syringes to be exchanged by adult males within Chicago neighborhoods. $15,000
Chicago Women's AIDS Project - to support the salaries of a full-time supervisor and a full-time program manager for the client services programs. $25,000
Christian Community Health Center - to support a manager of information systems position. $15,000
Community Supportive Living Systems - to provide general operating support for this southside supportive housing agency. $10,000
CORE Center - to provide support for a peer leadership program to assist new patients in accessing care services at the CORE Center. $15,000

Holy Cross Chruch - Immaculate Heart of Mary - to continue the HIV/AIDS Outreach Prevention Education (H.O.P.E.) program, offering social services and prevention outreach in the Back of the Yards neighborhood. $10,000
Horizons Community Services, Inc. - to provide general operating support for the Youth Program and Psychology Services. $10,000
Howard Area Community Center - to support the HIV prevention project serving communities of color in Rogers Park. $15,000
Howard Brown Health Center - to support individual and group HIV prevention education services for gay and bisexual men in Lakeview and Uptown. $10,000
Interfaith House - to support the HIV/AIDS Education and Services Project, which aims to reduce the risk of HIV infection among Chicago's homeless population and support residents who are living with HIV/AIDS. $10,000
Jelani Unified Men's Project (Greater Chicago Committee) - for operating support of this agency that provides prevention services to gay and bisexual men of color. $10,000
Lambda Legal Defense & Education Fund, Inc. - to support the AIDS Project, which provides advocacy services on behalf of people living with HIV/AIDS in the Chicago metropolitan area. $15,000
Legal Assistance Foundation of Chicago - to support legal services provided to people living with HIV/AIDS. $15,000
Near North Health Service Corporation - to provide culturally appropriate HIV prevention materials and condoms to clients through its health centers located throughout the city. $5,000
Open Door Clinic - to continue the delivery of primary medical care for people living with HIV in the collar counties. $20,000
Project VIDA - to support the HIV prevention and risk-reduction project targeting young African-American and Latino men. $15,000
Questioning Youth Center - to support HIV prevention programs targeting gay, bisexual, transgender, and questioning youth in DeKalb. $5,000
Regional CARE Association - for general operating support of this Joliet-based primary care organization. $15,000
Research & Education Foundation of Michael Reese - to provide medication adherence education and counseling services to clients of its HIVCare program. $20,000
Roger Baldwin Foundation/ACLU - to support statewide litigation, education, and public policy work aimed at ending discrimination and mistreatment of those affected by HIV and AIDS. $10,000
Sisters and Brothers Helping Each Other - to provide general operating support for this HIV prevention agency serving Will, Kankakee, and Grundy counties. $5,000
South Side Help Center - to support the Prevention for Positives Program targeting HIV-positive African-Americans. $15,000
TaskForce AIDS Prevention - to support outreach, education, and prevention services targeting HIV-positive gay and bisexual men of color. $10,000
Test Positive Aware Network - to expand distribution of its HIV/AIDS journals Positively Aware and Positively Aware en Español. $10,000
The Night Ministry - to support the Outreach and Health Ministry program staff, Health Outreach bus, and HIV prevention services. $10,000
University of Illinois Community Outreach Intervention Project - to provide syringe exchange and harm reduction services on the west side of Chicago. $15,000
University of Illinois HIV/AIDS Dental Program - to support the salary of a dentist and expand dental care services to clients living with HIV $10,000
University of Illinois HIV/AIDS Project - to support medical adherence and treatment services for minority HIV-infected populations at seven community-based outreach sites. $10,000
Vida/SIDA - The Puerto Rican Cultural Center - to support HIV counseling and testing, prevention, and risk-reduction education projects targeting Latino clients. $15,000
Vision House - for general operating support of this southside HIV/AIDS housing provider. $10,000
Vital Bridges - to provide food and nutrition services to people living with HIV/AIDS on Chicago’s south and west sides. $25,000
Westside Holistic Family Services - to support Project Decrease, which provides prevention outreach and education to high-risk populations on Chicago’s west side. $10,000
Women's Resource Assistance Program - to continue and expand community and street outreach, risk reduction, and HIV prevention services targeting high-risk women in Chicago’s south suburbs. $10,000
Working for Togetherness - to expand the Prevention Outreach for Positives program. $5,000
Young Women's Empowerment Project - to provide HIV prevention services to young, female sex workers. $5,000
SPECIAL INITIATIVES


Chicago Housing for Health Partnership - to support evaluation activities of this multi-agency partnership for which AFC serves as the lead agency. The Partnership is a three-year demonstration project that provides housing for chronically ill homeless individuals and evaluates the cost-benefits of a comprehensive array of housing and social services.

Perinatal HIV Case Management Project -
to provide intensive case management services to pregnant HIV-positive women. The project, which is funded through the Pediatric AIDS Chicago Prevention Initiative, aims to eliminate mother-to-child transmission of HIV by increasing women's participation in prenatal care. Sinai Health Systems and University of Chicago are the funded agencies providing case management services through this project.

Dance for Life -
to support the beneficiaries -- Chicago House, Project VIDA, and the Dance for Life Fund -- of the annual fundraiser Dance for Life. This grant is made possible by Chicago Dancers United.

AmeriCorps Program Matching Grant -
to support seven AmeriCorps members at seven different agencies throughout the Chicagoland area. The participating agencies are: AIDS Legal Council of Chicago, Chicago House & Social Service Agency, Chicago Recovery Alliance, Christian Community Health Center, Haymarket Center, Horizons Community Services, and Infant Welfare Society.

AmeriCorps members provide HIV prevention, care, and testing services as part of a year-long term of service. This grant is coordinated through the National AIDS Fund.
