Quality Management Coordinator

The AIDS Foundation of Chicago (AFC) mobilizes communities to create equity and justice for people living with and vulnerable to HIV and related chronic diseases. We envision a world in which people living with HIV and related chronic diseases will thrive, and new HIV infections will be rare. Our work focuses on improving health equity, preventing new cases of HIV, serving as a collaboration and knowledge center, and being a bold voice for change.

We are seeking for an individual to serve as our Quality Management Coordinator. The Quality Management Coordinator will assist in quality management activities for the housing and supportive service programs.  This position will work with housing team members, other key personnel and subcontracted partner agencies to implement policies and practices to ensure delivery of high-quality services to participants throughout AFC’s partnerships of supportive housing supportive services and housing assistance programs.  The Quality Management Coordinator will also be responsible for input of quality data, and coordinate quality assurance and improvement activities in the Client Track management information system (MIS).

Principal functions and responsibilities of this position include the following:

Quality Assurance

  • Work with the housing team to monitor quality improvement practices to ensure attainment and adherence to program standards;
  • Assist the housing leadership team in identifying best practices and implementation regarding housing programming;
  • Obtain and review client intake assessments and reassessments from partner agency case managers and follow-up on missing, incomplete, or incorrect data, as necessary;
  • Serve as the Agency Technical Administrator (ATA) for the Chicago Alliance’s Homeless Management Information System (HMIS) and conduct quarterly data checks.

 

Data Entry and Reporting

  • In conjunction with the Research, Evaluation and Data Services (REDS) team, complete and distribute monthly, quarterly and annual reports related to the housing program scopes of service and program standards;
  • In collaboration with Program Development, ensure Annual Report data is accurate and does necessary follow-up to obtain appropriate or missing data;
  • Input Supportive Housing Program intake and six-month reassessment data into ClientTrack/MIS.

 

Site Visits and Audits

  • Assist the Director of Quality Management and Compliance with coordinating logistics for annual partner agency site visits; provide supportive site visit and audit documentation for final reporting; assist with the monitoring of follow-up activities;
  • Assist the Director of Quality Management and Compliance in coordinating logistics for funder audits.

 

 

Other

  • Assist with agency-wide activities as directed, including Annual Meeting, AIDS Run & Walk, and others;
  • Protect organization's value and manage risk by keeping information confidential;
  • Perform other duties as assigned.

 

QUALIFICATIONS

 

Minimum Qualifications

Associate’s degree in Urban Studies, Sociology or related field

AND 2 years’ experience related to Housing, Health or Social Services

OR Bachelor’s degree in Sociology, Psychology, Urban Studies, Library & Information Sciences.  

 

Preferred Qualifications

Bachelor’s degree in Sociology, Psychology, Urban Studies, Library & Information Sciences

3 or more years’ experience related to contract management and compliance in Housing, Health or Social Services

1 or more years Quality Management experience

1 or more years database systems functionality experience (i.e., HMIS)

1 or more years report writing experience

 

Knowledge, Skills, and Abilities

Strong attention to detail and the ability to accomplish a task with concern for all areas involved, no matter how small.  This includes the ability to notice inconsistencies/faults in organizational processes in order to ensure the quality of services provided by the organization and to prevent further inefficiencies.  The ability to attend to and verify the accuracy and completeness of detailed information in documents or on the computer, such as charges, fees, due dates, etc.

Knowledge of client-level data collection systems (i.e., HMIS, Client Track) and other database programs.  This includes the ability to enter, extract, and track data using these programs.

Strong knowledge of quality control policies and procedures. The ability to breakdown complex processes to identify quality improvements and put measurements in place.

Also, strong verbal and written communication skills and ability to work with diverse populations are key to success in this role.
