Case Management
Request Case Management
AFC’s case management program connects individuals to vital resources such as treatment, testing, food, financial support and housing.
Contact us at 312-690-8860 to request case management services. One of AFC’s program staff will refer you to an agency in your area that will provide you with a case manager who can help you get the services you need. Your case manager may also be able to link you with additional services not listed here. If you need, case management please call 312-690-8860 or complete the following application. The application will be routed directly to AFC.
Please note: Due to confidentiality, only the person seeking services can enroll as a client in the case management system. Family members, friends or social workers cannot enroll someone else for services without formal verbal or written consent.
Case managers help people living with HIV obtain:
- Medical and home health care
- Dental services
- Assistance enrolling into medication assistance programs
- Assistance processing the payment of HIV related medical out of pocket costs for eligible client
- Substance abuse and mental health treatments
- Legal services
- Access to food pantries
- Public benefits
- Housing options
- Other support programs
- Additional services through special programs
The eligibility requirements for most of our HIV case management programs are:
- Proof of HIV diagnoses
- Proof of residence in the Chicago Eligible Metropolitan Area (EMA)
- Proof of income at or below 500% FPL OR $62,450 for a family size of 1
Justice Involved / Corrections Case Management:
AFC and other organizations coordinate a system of services for individuals living with HIV that are justice involved (recently released from jail or prison). These individuals receive primary medical care, substance use services and/or mental health services, medication assistance, transportation to medical appointments, legal services, support groups, emergency rent and utility assistance, assistance with finding employment, affordable housing, and other support services as needs arise. Through grantmaking and partnerships, AFC leads systems of housing and care including more than 50 partner organizations. AFC partners are critical to ending the HIV epidemic and homelessness. Learn more about our grantmaking and partnerships!
HIV Care Services
The AFC case management system provides a wide range of services such as connections to health care, transportation, housing and food support to more than 6,000 individuals each year.
