From its inception, AFC has led city-wide efforts to coordinate essential medical and support services for people living with HIV/AIDS.
With initial funding from the Health Resources and Services Administration (HRSA), AFC established a coordinated system of case management, framed by a set of common expectations, policies and procedures that structure the provision of services.
The AFC case management system provides a wide range of services, acting as a broker between clients and services. Our system identifies medical and social service needs and facilitates referrals to meet those needs; assists clients in applying for benefits, including housing, transportation, food and nutrition services; and facilitates access to emergency homelessness prevention funds in times of crisis. AFC has also invested significantly in a network of providers offering intensive case management and supportive housing options for people living with a wide range of chronic illnesses.
