Grantmaking and Partnerships – Case Management

Established in 1989 as the Northeastern Illinois Case Management Collaborative (the Collaborative), the Collaborative coordinates high-quality, cost-effective Ryan White HIV/AIDS Program services in Cook County and surrounding Collar counties. Most recent award amounts totalled $8.82 million. Awards agencies are subcontracted for a minimum of three years unless an agency fails to meet service delivery requirements. Funding levels are based on identified needs and service gaps, as well as current service utilization and projected gaps in services.

Our 2022 grants for Ryan White Case Management

AFC’s case management program continues to address the unmet health needs of more than 6,000 persons living with HIV and AIDS. About 86% of people in AFC’s Ryan White case management program are virally suppressed.

Viral suppression supports one’s individual and community health, as someone who is virally suppressed is healthier and cannot pass HIV sexually to their partners – this is known as U=U or Undetectable=Untransmittable.
