AFC’s Certified application counselors (CACs) can help people complete Medicaid, Medicare, Marketplace, and off-exchange applications.  AFC CACs build awareness and advocacy about Medicaid eligibility and redetermination. 

There is no deadline for Medicaid enrollment. CACs can help with enrollment in Medicare October 15 through December 7. Affordable Care Act marketplace plan enrollment is available from November 1 through December 15. 

Enrollment can happen in person or remotely, and bilingual staff and translation services are available. 

Community collaboration

If you are a community leader or staff member at a Chicago-based agency or organization, AFC and our program partners can help educate and enroll your clients into new health care options. Our medical benefits specialists can provide educational presentations and enrollment services to your clients at your agency or a community/neighborhood location or online.
