Permanent Supportive Housing

Supportive Housing is designed for people who are homeless and living with a disability. It is 1) affordable rent connected with 2) wraparound case management services 3) that has no time limit on how long you can stay in the program.

AFC Housing and Utility Assistance Program

If you are in need of one-time housing financial assistance, call the City of Chicago (311) and ask for short-term help. You will then be screened for financial assistance eligibility and assisted through the application process.

If you don't have a case manager, please contact one of AFC's intake coordinators at 312-690-8860.

Already have a Ryan White or DRS case manager? Contact them to request application assistance for the AFC Housing and Utility Assistance Program.
